Voluntary Participation: I am voluntarily participating in a Pilates Matwork session offered by [Your Name]. I understand that Pilates involves physical movement and exercise, and as with any physical activity, there is a risk of injury. Medical Clearance: I confirm that I am physically able to participate in this class. I have either received clearance from my healthcare provider to engage in physical exercise, or I accept full responsibility for participating without such clearance. I have disclosed any injuries, medical conditions, or limitations that may affect my ability to participate. Assumption of Risk: I understand that participation in this class is at my own risk. I am aware that physical activity may cause injury or aggravate existing conditions, and I voluntarily accept all risks associated with my participation. Release of Liability: I release and discharge Jessica Norman and any associated parties from any and all claims, demands, or causes of action arising from injury, illness, or damage that may occur during or as a result of my participation in this Pilates class, whether caused by negligence or otherwise. Personal Responsibility: I agree to listen to my body, modify exercises as needed, and immediately inform the instructor if I experience pain, discomfort, or unusual symptoms. I understand I am responsible for my own well-being during class. Use of Props and Equipment: I understand that I may be asked to use simple props or equipment (e.g. mats, blocks, straps), and I agree to use them safely and as instructed.
